What Treatment in India Actually Costs — and What Estimates Leave Out

Families rarely get an unpleasant surprise because Indian hospitals are expensive. They get one because they compared a package price against an itemised estimate without realising the two documents cover different things.
How an Indian hospital builds its estimate
A hospital estimate is built on an assumed clinical pathway: a defined procedure, a stated number of ward days, a stated number of ICU days and a standard set of investigations. Change any one of those assumptions and the number changes.
That is why an estimate is a good-faith projection rather than a fixed price. Reputable hospitals state this on the document itself.
What a typical Indian surgical estimate is made of

What is usually included
Most surgical packages cover:
- Surgeon, anaesthetist and operating theatre charges
- A specified number of ward and ICU days
- Routine post-operative investigations and in-hospital medication
- Standard implants or consumables where the package names them
What is usually excluded
The items that move a final bill are almost always these:
- Additional ICU days beyond the packaged number
- Premium implants, stents or valves chosen over the standard option
- Treating a complication or a pre-existing condition discovered during workup
- Blood products, specialised antibiotics and extended physiotherapy
- Everything outside the hospital: accommodation, food for attendants, local transport
Comparing two or three quotes fairly
Put the estimates side by side and normalise them before you look at the total. Check that each assumes the same procedure and approach, the same implant grade and a comparable number of ICU days, and confirm whether the named surgeon is the one who will operate.
A quote that is 20 percent lower with half the ICU allowance is not cheaper; it is a different assumption about your recovery.
Budgeting beyond the hospital bill
Plan for accommodation near the hospital for the attendant during admission and for the recovery period after discharge, local transport, SIM and forex, the follow-up review before flying, and a contingency of roughly 15 to 20 percent of the hospital estimate.
